It's only worth getting travel insurance if you have to use it.
If you live in the USA or Canada, you can get Trip Cancellation / Interruption Insurance plans that include emergency medical / evacuation coverage plus they will reimburse you for the money you lose if your doctor advises you against travel because of a sickness or injury.
You can also cover pre-existing conditions.

Definately, A friend of mine got robbed in Paris Tickets,camera passport cards and money ,if they didnt have insurance they would have been stuck there. They got compensated for everything. Not sure which Insurance Company.
Absolutely.....not only for some of the reasons mentioned by others. But I can almost all but guarantee you that you aren't medically covered internationally. Most aren't for reasons that well not everyone leaves the state, let alone the country and if they covered people for this then premiums would only go up.

Depending on when your travel was purchased will determine if you would be covered for pre-existing medical conditions if you or any travel companions had any that is.
I would recommend getting travel insurance if you are making multiple airline and/or train connections. There are many things that can go wrong and cause you to miss one or to lose luggage.

definately. There are many companies to choose from, I like CSA travel insurance and trip guard myself.
